Community Health & Medical Support Initiative
Purpose and Objectives
Improve overall community health and wellbeing through a mix of preventive, promotive, and curative services delivered at or near where people live.
Reduce avoidable illness, disability, and out‑of‑pocket health expenditure by offering free or low‑cost camps, screenings, medicines, and referrals.
Ensure no one is left behind in access to basic healthcare, particularly women, children, seniors, and people with chronic conditions or disabilities.
Core Health Services
Free community health camps and outreach clinics providing primary consultations, vital‑sign checks, and basic diagnostics (BP, sugar, hemoglobin, etc.).
Health screenings for common conditions such as vision and hearing problems, anemias, malnutrition, diabetes, hypertension, and other disabling disorders.
Distribution of essential medicines and supplements, along with counselling on correct usage, adherence, and follow‑up care.
Preventive and Promotive Activities
Health education sessions on nutrition, malnutrition and anemias reduction, maternal and child health, sanitation, and hygiene practices.
Counselling to discourage early marriage, promote spacing of births, exclusive breastfeeding, and registration of pregnant women at health center's.
Support for immunization and other public‑health measures by linking families to government programmers and local health and wellness center's.
Community‑Based Care and Linkages
Engagement of community health workers, volunteers, and local caregivers to bridge gaps between formal health facilities and households.
Use of mobile diagnostic vans or outreach units to reach remote or tribal areas with limited infrastructure, offering periodic camps and referrals.
Strong referral pathways to designated hospitals, specialty center's, and government schemes for cases needing advanced or long‑term treatment.
Inclusion and Target Groups
Focus on backward, tribal, and underserved communities where health services are scarce and disability or chronic illness is common.
Special attention to mothers and children, including programmers to reduce childhood diseases, malnutrition, and preventable disabilities.
Prioritizing women’s health, older persons, and people with disabilities as key beneficiaries of screenings, rehabilitation, and entitlements support.
Expected Outcomes
Reduction in existing disabilities and incidence of future disabilities through early detection, timely treatment, and strong preventive education.
Better access to comprehensive primary health care at community level, aligned with national initiatives and health‑and‑wellness Centre frameworks.
More empowered communities that understand health risks, use services proactively, and participate as co‑providers and stewards of local health systems.
